Our homes serve as a bridge between hospital and independent living, supporting individuals who need continued recovery time but do not require hospitalization.

HOW IT WORKS:
• Patient is medically cleared for discharge • Patient is placed in our structured residential housing • Licensed home health providers deliver care within the home (if applicable) • Patient recovers in a safe, stable environment
IDEAL CANDIDATES:
• Medically stable for discharge
• Independent with activities of daily living
• May require continued recovery (IV therapy, wound care, etc.)
• No active infectious conditions (including COVID-19)
• Psychiatrically stable
• Does not require 24/7 skilled nursing or detox
